In an effort to increase student satisfaction and to provide consistent and accurate information to students, please complete this form if you have heard recently that a student/s has received contradictory information from somewhere on campus. This form will be submitted anonymously to Robert Cox, Dean for Student Success, and he will investigate the matter. Information submitted in this form is used for quality assurance purposes only.
