How To Receive Accommodations Through The

College Placement Test
Do you need accommodations for the College Placement Test? If you require special testing accommodation, contact the Disability Services Office (DSO) at 503-491-6923 to make arrangements. You will need to supply us with documentation of your disability that supports your request for accommodations. |

Application For Disability Services
Call or stop by the Disability Services Office (DSO), to make an appointment with the Program Advisor for application for services, telephone: 503-491-6923, v/tdd: 503-491-7670, Fax: 503-491-7549. |

Attend your appointment with the DSO Program Advisor. Bring your documentation with you. Documentation for a medical or psychological disability must be no more than 1 year old. Documentation for a learning disability must be no older than 3 years (or have been done during your senior year of high school, or at age 18 in order to meet the minimum criteria for testing done as an adult). During this appointment you will apply for accommodations, learn about registration procedures and college policies, receive academic advising, and register for classes. Expect to spend an hour to an hour and a half. |

When you have completed your application with the program advisor, we will make an appointment for you to meet with the DSO Coordinator. In this meeting, you will learn what accommodations you qualify for. |

If you have questions about financial aid, you should direct your questions to the Financial Aid Office: 503-491-7262, or visit the website. If Vocational Rehabilitation is paying for your tuition and fees, written authorization must be on file in the Business Office when you register, otherwise, you will be billed. Your tuition and fees must be paid by the first day of the quarter you are registering for. When you register for a class you incur charges and are responsible for payment of the charges for the class, whether or not you attend. If you have any questions about this, please talk with your DSO Advisor. |

It is the student’s responsibility to inform the DSO each term, of the classes you are taking, and accommodations you are requesting two weeks in advance. This gives us time to arrange the accommodations. We will make Instructor Notification forms for you so you can discuss the arrangements with your instructor on the first day of class. |
