Instructions for returning DSO approved students who have previously received services through the DSO office.
Go to the next page "MT. HOOD DISABILITY SERVICES: STUDENT ACCOMMODATION REQUEST FORM". Fill out the form with your Internet Browser by either clicking or keyboarding to the areas with your information.
1. Include your Name, Date, Term and choose Year.
2. Fill out the Courses, Section, and Instructor areas.
3. You may choose "I want the same accommodations I had last term".
4. If you are requesting New accommodations, fill out this part.
5. Put in your Student ID Number.
Remember DSO determines the final approval of the accommodation!
