Project YESS Documentation Check Sheet

This information will be needed to verify eligibility for Project YESS. If you are unable to get documentation for any of the categories listed below, let us know before your scheduled appointment. Please call 503.491.7641 (FAX 503.491.6016). You will need state issued photo ID and, if under age 18, a high school referral or release before you can begin GED classes.

Do you have the following documents?

Social Security Number (need one of these documents):

	Original Social Security Card
	Employment Check Stub/W-2
	Oregon Health Plan Medical Card

Date of Birth (need one of these documents):

	Original Birth Certificate
	Government issued photo ID card
	Public Assistance Record

Proof of Citizen/Legal Resident (need one of these documents):

	Original Birth Certificate
	Original U.S. Passport
	Public Assistance Record
	Original Alien Registration Card/ INS Authorization
	Oregon Heath Plan Medical Card

Proof of Address(need one of these):

	State ID card with current address
	Postmarked mail addressed to applicant

Proof of income for the past six (6) months (need all that apply to you):

	Pay check stubs
	Cash welfare grant authorization
	Medical card showing cash grant status
	Benefits Letter (Social Security, SSI, Pension, etc.)
